Green Meadows Rehabilitation and Nursing Center

I'm not sure that there is a business that should be out of business more than this business. The conditions are deplorable. The staff has zero intelligence. The food is inedible. It truly seems like a place where elderly who have no one are left to die alone. trying to find someone to help your loved one in a timely manner is just about impossible. The only decent staff member was Cory, the physical therapist. Elementary students could perform better than anyone else we met. If you love your loved one, do not send them here.

I am caring for my single Aunt at home now after 12 hours in this uncaring , dirty facility. Any hospital or Doctor that recommends this facility should send a loved one there and stay there themselves. All humans need to be treated with dignity especially those put on hospice. Its not for my family that im writing this review, my aunt is safe. Im writing it for all the beautiful patients still there with no one to check on them and for anyone considering this place.

If I could give this place no stars I would. My brother was here for physical therapy rehab from injury. Let's just say the staffing issues are the worst I have seen in a long time. They use temp staff and they don't care about the patients care what so ever. Meds are late patients care ignored. My brother was there a week with no shower because he was in a covid lock down room so couldn't go to shower even without having covid. The place is dirty run down not taken care off a waste of money for anyone to go there. And the office people do not try and help with your complaints. When you hear nurses say a patient is yelling I can't breathe and another says let her die one less to care for that's the kind of care you get here. No one should ever be forced to go here.

This place needs to be closed down. On top of the complete neglect of all the residents and the complete filth of the place, only one elevator was working in the 2 months a family member was there. The other one elevator took forever and the stairwell was locked. If there is a fire I can't imagine what would happen. The people who work here and own this place should be ashamed of themselves I can't imagine they would want their family members treated in such a deplorable and inhuman way.

Reading these reviews makes me want to cry. My mother has been here for two years. I've done everything but strip naked and hold a sign on rt 30 to try and get decent care for her. I agree with everything and so much more!! Lots of pictures from two years to prove it! I've bathed my mother and cleaned her room on my hands and knees. Some staff extremely rude!! Some staff have been great and caring towards my mother!! It's not everyone. While there's been little improvement and have little hope of things improving enough to make me feel okay about her being there. I will continue to bathe her and clean her room!! She will not go down like that!! She has dementia. She's taking being there better than me! I will continue to do what I can and I'm not afraid to use my voice!! (Also not worried about the cops!!) Administration is a joke and assistant social worker is a bully and of little to no help. On and on we go!! Thanks everyone else for sharing!!

Disgusting, smelly, dirty, unsanitary. Overworked, understaffed, miserable employees. Management could care less about the patients walking around in soiled clothes, dirty diapers, vomit on clothing, feces on their pants, the horrific smell is unbearable.

Try to find a staff member when you need one. GOOD LUCK WITH THAT. There will be 10 people at the desk chatting it up while the patients wander the halls like zombies.

I took my mother out of this rat hole and am filing a long, detailed complaint to make sure that they stop ignoring their patients. For the money we paid.... they ROBBED my poor mother and she declined probably 7 years within the few months she was there.

Don't fall for the pitches they give you. The sweet, low tone of voice trying to sound caring. They DON'T CARE!!!!!!! Go ANYWHERE else but DON'T put a loved one in this dump!

Food from the 80's coats the floor of the dining hall.
They don't do basic necessities for the patients, brush teeth, hair, clean, make beds, NOTHING.

I am disgusted that they are able to get away with what they are doing.

Green Meadows... Another name change same horrible practices. Don't be fooled. Beware of their sales girl Eileen who boasts about the great care you will receive here. They are understaffed and most of the staff are miserable uncaring people. Too many chiefs and not enough Indians here. Management needs to roll up their sleeves and help out and not parade around lying to the residents telling them how well they are being treated. This place is nothing but a medicare and insurance cash grab for the owners. I will say the physical therapy dept is good, but that doesn't make up for the rest of the issues here. This place should be bulldozed and most of the employees banned from working in any kind of care profession.

Bottom line: Do yourself a favor here and recover at home by yourself. You will get better care there.

My father went to this center and will never return!! When I walked into the center for the first time I didn't get a good reception. The lady at the desk acted like I was bothering her when I asked where I sign in because it was my first time. Then I asked her where the elevator was and she just pointed. I found my way to my fathers room and he just had surgery and wasn't allowed to put any weight on his foot. He was at this facility for intense rehab but when I went to his physical therapy session it was a joke. He could have done those exercises at home.

We were instructed to bring in a list of his medication so that they could continue his regular meds. They had him on medication that he stopped taking for years so he was over medicated and passing out because of it. We had never even met his Doctor and someone was with him everyday. It seemed like they just wanted the money for his stay and didn't care about the patient. I vehemently don't recommend this facility to anyone and ask family members to regularly check up on your loved ones at this facility. My Mom signed my Dad out and is doing better at home.

i would not even give this place a 1 star rating.please do not send your loved one to this facility.! the staff does not have basic nursing knowledge.did not recognize signs of infection, deteriation of condition of my loved one even after family concerns were told to nurses many times. nurses do not communicate with staff. at shift change, nurses not given report on their patients. unaware of patient history, capabilities, and needs. had to update each new nurse myself. cnas were bothered when asking for assistance. bed sheets soiled and not changed daily. had to ask for clean sheets which were left on the dresser, family changed bed. my elderly mother and i did nursing care, repositioning, feeding, going to bathroom etc. malnurished- tray was dropped off. no assistance with feeding. we fed him. served inappropriate foods for his care. facility is filthy. family brough in cleaning supplies and cleaned bathroom. room was not cleaned at all during stay, until i complained.

i called facility physician that my dad was deterorating, did not listen to my plea to admit my father to the hospital. i contacted my dads primary doctor and explained concerns, who was insistant he go to hopsital right away. we planned to call 911 ourselves but that same morning my dad complained of chest pain left via ambulance, admitted to hospital, ended up in icu, spent 7 days in the hosptial. (sufficient medical care?) he never offered to go see him. simply said my dad was receiving sufficent medical care and he was intouch with nurses daily. sufficent medical care does not warrent a
7 day admission to the hospital.

administrator was visibley annoyed and making excuses for facility failure to care for my dad. i demanded he be moved elsewhere, but got sick before that happened.

iv seen a woman parked at nurses station crying, asking for help, said she was so thirsty and asking for water. staff ingnored her until i told them this woman needs water. 1st floor, i heard residents crying out in pain,asking for help. while nurses sat at nurses station, didn't move a muscle to check on resident. heard a staff memeber yelling at a resident for ringing the call bell.

my dad has family and we advocate for him but alot of resident dont have a voice or family to protect them which is heartbreaking that our elderly would be sent to a place like this. please do not send your family or friend to this facility

. i would strongly urge the pa department of health to shut this place down. no more chances to correct violations!. exaclty who is protecting our elderly from this facility? allowing this place to stay open is causing serious harm to the residents there, and unecessary suffering

. im not staying quiet or going away. i will negatively review this facility every opportunity i get.

my dad is in a wonderful facility now and in just 5 days, he has gained strenth, recieving proper medical attention, he is clean, being fed. happy. he is improving each day.
